2009 MGS Recreation League Registration (FAST PITCH GIRLS SOFTBALL)
Registration Options
There are 3 ways you may register.
(1) in person (Feb. 7 & Feb. 14)
In-person registration will take place the first two Saturdays in February from 9 AM to 2 PM at the Harmony Grange (3301 Limestone Road). If you registered last year we plan to provide you a copy of your registration information from last year. Alternatively, you may complete and bring the two forms below. In addition, registration fees must be paid by a check made payable to "Midway Girls Softball".
(2) mail in
Complete and mail in BOTH forms below along with a check made payable to
"Midway Girls Softball".

(3) on-line (through Feb. 20) Use "Pay Now" buttons below.
Pay on-line with
PayPal or a credit card. After you pay you will then be asked to complete a
registration form on-line (ONE FOR EACH PLAYER). So
you do NOT need to complete the Registration Form below. In addition,
a signed Appointment
of Agent (waiver) must be provided. This can be given to your
coach (MGS must have the
signed form before the first practice). On-line payments include
a $5 convenience charge.
NOTE: All players moving to a new age group or new to the league must attend evaluations on February 28 (excludes 6U and 8U).
No matter which way you register, you need to provide 3 things:
- Registration

Registration Fees (*): Maximum fee of $250 per household.
| One Player (8U, 10U, 13U, 14+) | $125 | |
| One Player (6 & Under) | $85 | |
| Two siblings | $205 | |
| Two siblings (both 6 & Under) | $165 | |
| Three or more siblings | $250 |
(*) This registration fee includes a $25 participation fee
per household which is refundable upon completion of a board-approved volunteer
activity. To receive a refund the participant must (1) complete
the activity, (2) complete the
Participation Fee Refund Form form which must be signed by a board
member, and (3) mail the form to the MGS address listed on the form by
July 31.
Payment and forms must be received by February 28 or a late fee will
apply (6U, 8U, 10U, and 13U age groups). The last day for
registrations in the 14+ age group is March 7. Incomplete mail in registrations may be returned to you.
