As with any participation in athletics, it is Midway Girls Softball's obligation to alert you to the possibility that an accident may occur leading to an injury that may cause devastating results. Please read and acknowledge with your signature the following statements:

I hereby appoint a representative of Midway Girls Softball, of legal age, as my agent and representative for the purposes of authorizing and consenting to hospital care and/or medical treatment of my child, _________________________, for any illness or injury that may occur while she is in the care or custody of the agent during the 2007 Recreation season, while I am away on vacation or otherwise not immediately available to give such consent.

I agree to hold harmless the organizers, sponsors, participants and persons transporting my daughter to or from the activities of this league, for any claim arising out of the injury to my daughter, whether the result of negligence or any cause, except to the extent and in the amount covered by accident or liability insurance.
